Double down and doing great! Heidi Montag made her first red carpet appearance after undergoing breast reduction surgery last October.

The 28-year-old Hills alum marked her one-year after surgery at the 10th anniversary party of a tabloid mag on Thursday, Oct. 23. For the Hollywood event, the star stepped out in a fitted black sweater tucked into a leather skirt. Montag left her trademark blonde hair down in pretty waves and accessorized her outfit with strappy sandals and a gold clutch.

Most surprising of all was Montag’s significantly smaller bust, making her look more like her original self prior to the 10 surgeries she underwent back in 2010. Spencer Pratt‘s wife beamed and looked comfortable for her entree back on the red carpet Thursday evening.

In November 2013, Montag opened up to Us Weekly about the problematic side effects of her F-cup boob implants, which weighed three pounds each. “I couldn’t conceptualize the weight of them in my body,” she told Us. “They felt like bowling balls on my chest. I felt inappropriate. Guys would stare at me really creepily and felt they had the right to because I had such big boobs.”

The reality star — who called herself “the new, new, new Heidi!” after the procedure — told Us that her doctor gave her a terrifying prognosis which prompted her to reduce her breast size. “He told me they were falling through the bottom, and I got scared,” Montag confessed. “It’s super dangerous. They can fall through to your belly button!”

The star’s latest red carpet appearance coincided with her Extra interview from Thursday, where she discussed Renee Zellweger‘s recent transformation. “I don’t know if Renee Zellweger just aged like she said, I don’t really know,” she said. “For me, when I see people who look totally different, I have an empathy. I feel like I know why at least I did it, and it kind of brings it back to that time in my life. It’s just, I feel for them.”

She also touched on making more public appearances after her own reclusive behavior post-surgery. “You can’t hide forever,” Montag said to Extra. “If you’re a public figure, you have to come out eventually. You have to feel good and feel confident about it.”
